{"id":1727,"date":"2025-09-17T08:03:28","date_gmt":"2025-09-17T08:03:28","guid":{"rendered":"https:\/\/curtin.edu.my\/student\/?page_id=1727"},"modified":"2025-09-17T08:03:30","modified_gmt":"2025-09-17T08:03:30","slug":"curtin-access-plan","status":"publish","type":"page","link":"https:\/\/curtin.edu.my\/student\/health-wellbeing\/curtin-access-plan\/","title":{"rendered":"Curtin Access Plan"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">A Curtin Access Plan (CAP) is a document that outlines the reasonable adjustments recommended to support students with disabilities, physical or mental health conditions or a carer of a person with disability to enable equitable access to their studies and assessments.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The counsellor will discuss the content of the CAP with you and it will only include information that you agree to release. It can include information about your condition if you wish to disclose this information.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A CAP has an end date after which it is not valid. This date is determined by the counsellor depending on a number of factors. In some cases it is valid for the duration of your course.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is your responsibility to provide a copy of your CAP to your unit coordinators and unit lecturers at the beginning of each semester.<\/p>\n\n\n<section\n                class=\"block accordions\" role=\"complementary\"\n        aria-label=\"Accordion\"\n        data-segment=\"all\">\n\n        \n\n    <div class=\"accordions__container\">\n                    <div class=\"accordion\">\n            <button class=\"accordion__heading accordion__toggle\" aria-controls=\"am-i-eligible-for-a-curtin-access-plan\" aria-expanded=\"false\">Am I eligible for a Curtin Access Plan?<span class=\"ico ico-chevron-fat ico--deep-blue\"><svg xmlns=\"http:\/\/www.w3.org\/2000\/svg\" viewBox=\"0 0 32 32\"><path d=\"M10.91 30.41l-2.82-2.82L19.67 16 8.09 4.41l2.82-2.82L25.33 16 10.91 30.41z\"\/><\/svg><\/span><\/button>\n        <div class=\"accordion__content content\" id=\"am-i-eligible-for-a-curtin-access-plan\" aria-hidden=\"true\">\n                    <p>You are eligible for a CAP if:<\/p>\n<p>\u2022 You have a diagnosed disability, physical or mental health condition<br \/>\n\u2022 You have a temporary disability such as fractures and injuries<br \/>\n\u2022 You have significant caring responsibilities of a person with disability<\/p>\n<p>If you are unsure of your eligibility, please reach out to counsellor to discuss<\/p>\n\n            <\/div>\n<\/div>                    <div class=\"accordion\">\n            <button class=\"accordion__heading accordion__toggle\" aria-controls=\"how-do-i-develop-a-curtin-access-plan\" aria-expanded=\"false\">How do I develop a Curtin Access Plan?<span class=\"ico ico-chevron-fat ico--deep-blue\"><svg xmlns=\"http:\/\/www.w3.org\/2000\/svg\" viewBox=\"0 0 32 32\"><path d=\"M10.91 30.41l-2.82-2.82L19.67 16 8.09 4.41l2.82-2.82L25.33 16 10.91 30.41z\"\/><\/svg><\/span><\/button>\n        <div class=\"accordion__content content\" id=\"how-do-i-develop-a-curtin-access-plan\" aria-hidden=\"true\">\n                    <p>Make an appointment with a counsellor to discuss your situation and the support you may need at Curtin.<\/p>\n<p><a href=\"https:\/\/curtin.edu.my\/student\/health-wellbeing\/request-counselling-appointment\/\">Request for the appointment online<\/a> or meet us in person at our office located on the ground floor of Prinia 1.<\/p>\n<p>Please provide a letter or statement from an appropriate qualified health professional which provides information about your disability or medical condition and how it impacts on your studies. We may require updated documentation when your CAP is due for review or your health circumstances change.<\/p>\n<p>Once you\u2019ve discussed your situation with the counsellor and provided the required health professional documentation, your CAP will be developed or updated in consultation with you.<\/p>\n\n            <\/div>\n<\/div>                    <div class=\"accordion\">\n            <button class=\"accordion__heading accordion__toggle\" aria-controls=\"how-current-does-your-documentation-need-to-be\" aria-expanded=\"false\">How current does your documentation need to be?<span class=\"ico ico-chevron-fat ico--deep-blue\"><svg xmlns=\"http:\/\/www.w3.org\/2000\/svg\" viewBox=\"0 0 32 32\"><path d=\"M10.91 30.41l-2.82-2.82L19.67 16 8.09 4.41l2.82-2.82L25.33 16 10.91 30.41z\"\/><\/svg><\/span><\/button>\n        <div class=\"accordion__content content\" id=\"how-current-does-your-documentation-need-to-be\" aria-hidden=\"true\">\n                    <p>Health professional documentation must meet the following currency requirements:<\/p>\n<p>\u2022 Temporary or changing conditions: You may be required to submit new documentation each semester or when your Curtin Access Plan expires.<br \/>\n\u2022 Permanent conditions: You must submit documentation initially, and thereafter only if your condition or support needs change.<br \/>\n\u2022 Learning disabilities: Your documentation must be dated within the past three years if the assessment was conducted before you turned 16. If the assessment was conducted after you turned 16 it should be valid for the duration of your studies.<\/p>\n<p>Counsellor will make the final decision on the suitability of your documentation and how often it should be updated.<\/p>\n\n            <\/div>\n<\/div>                    <div class=\"accordion\">\n            <button class=\"accordion__heading accordion__toggle\" aria-controls=\"how-is-my-curtin-access-plan-distributed\" aria-expanded=\"false\">How is my Curtin Access Plan distributed?<span class=\"ico ico-chevron-fat ico--deep-blue\"><svg xmlns=\"http:\/\/www.w3.org\/2000\/svg\" viewBox=\"0 0 32 32\"><path d=\"M10.91 30.41l-2.82-2.82L19.67 16 8.09 4.41l2.82-2.82L25.33 16 10.91 30.41z\"\/><\/svg><\/span><\/button>\n        <div class=\"accordion__content content\" id=\"how-is-my-curtin-access-plan-distributed\" aria-hidden=\"true\">\n                    <p>You will be emailed a PDF version of your CAP by your counsellor once it is finalised. A hard copy may be collected from our office during office hours.<\/p>\n<p>It is your responsibility to distribute your CAP to the relevant unit coordinators and the unit lecturers at the beginning of the semester whenever your CAP is issued or renewed.<\/p>\n<p>You will be notified by the unit coordinators or unit lecturers of how the reasonable adjustments can be applied to the unit. Please do not assume these adjustments have been applied until you have had written confirmation from the unit coordinator.<\/p>\n<p>While your confidentiality is respected, your reasonable adjustments outlined in your Curtin Access Plan (CAP) may be viewable by other relevant Curtin staff.<\/p>\n\n            <\/div>\n<\/div>                    <div class=\"accordion\">\n            <button class=\"accordion__heading accordion__toggle\" aria-controls=\"do-i-have-to-disclose-my-disability-in-the-curtin-access-plan\" aria-expanded=\"false\">Do I have to disclose my disability in the Curtin Access Plan?<span class=\"ico ico-chevron-fat ico--deep-blue\"><svg xmlns=\"http:\/\/www.w3.org\/2000\/svg\" viewBox=\"0 0 32 32\"><path d=\"M10.91 30.41l-2.82-2.82L19.67 16 8.09 4.41l2.82-2.82L25.33 16 10.91 30.41z\"\/><\/svg><\/span><\/button>\n        <div class=\"accordion__content content\" id=\"do-i-have-to-disclose-my-disability-in-the-curtin-access-plan\" aria-hidden=\"true\">\n                    <p>Your CAP will only contain disability information that you choose to release, and your disability or medical information\/health professional documentation will be kept confidentially by the Health, Counselling and Disability Services in accordance with our confidentiality policy. There are some exceptions to the boundaries of confidentiality which are outlined in the Informed Consent Form.<\/p>\n<p>Please discuss with your counsellor for assistance with your decision whether or not to disclose personal information.<\/p>\n\n            <\/div>\n<\/div>                    <div class=\"accordion\">\n            <button class=\"accordion__heading accordion__toggle\" aria-controls=\"what-happen-to-my-curtin-access-plan-if-my-situation-changes\" aria-expanded=\"false\">What happen to my Curtin Access Plan if my situation changes?<span class=\"ico ico-chevron-fat ico--deep-blue\"><svg xmlns=\"http:\/\/www.w3.org\/2000\/svg\" viewBox=\"0 0 32 32\"><path d=\"M10.91 30.41l-2.82-2.82L19.67 16 8.09 4.41l2.82-2.82L25.33 16 10.91 30.41z\"\/><\/svg><\/span><\/button>\n        <div class=\"accordion__content content\" id=\"what-happen-to-my-curtin-access-plan-if-my-situation-changes\" aria-hidden=\"true\">\n                    <p>You can contact a counsellor at any time to arrange a review of your CAP if the impact of your disability or medical condition changes, and\/or if the CAP is no longer meeting your needs. Updated health professional documentation may be required to support any changes.<\/p>\n<p>Unless you advise of the need to make changes or review your situation, your CAP will stay in place until the review date specified on the plan.<\/p>\n\n            <\/div>\n<\/div>                    <div class=\"accordion\">\n            <button class=\"accordion__heading accordion__toggle\" aria-controls=\"how-do-i-arrange-exams-using-curtin-access-plan-if-my-situation-changes\" aria-expanded=\"false\">How do I arrange exams using Curtin Access Plan if my situation changes?<span class=\"ico ico-chevron-fat ico--deep-blue\"><svg xmlns=\"http:\/\/www.w3.org\/2000\/svg\" viewBox=\"0 0 32 32\"><path d=\"M10.91 30.41l-2.82-2.82L19.67 16 8.09 4.41l2.82-2.82L25.33 16 10.91 30.41z\"\/><\/svg><\/span><\/button>\n        <div class=\"accordion__content content\" id=\"how-do-i-arrange-exams-using-curtin-access-plan-if-my-situation-changes\" aria-hidden=\"true\">\n                    <p>The CAP can be used to make alternative test or examination arrangements.<\/p>\n<p>School-based tests\/exams: You are required to inform your unit lecturer and the counsellor no later than two weeks prior to the examination to allow adequate time for the necessary arrangements to be made. Please note that arrangements may not be possible without sufficient notice.<\/p>\n<p>End of semester exams: These exams are usually run by the Examinations Office. Your exam arrangements will be valid for the duration of your CAP. Please contact your counsellor three weeks prior to the start of the examination period if you wish to request adjustments to your exam arrangements.<\/p>\n<p>A CAP cannot be used to apply for an examination deferment. Separate relevant supporting documentation is required if you wish to apply for deferment.<\/p>\n\n            <\/div>\n<\/div>                    <div class=\"accordion\">\n            <button class=\"accordion__heading accordion__toggle\" aria-controls=\"how-does-my-cap-relate-to-extensions\" aria-expanded=\"false\">How does my CAP relate to extensions?<span class=\"ico ico-chevron-fat ico--deep-blue\"><svg xmlns=\"http:\/\/www.w3.org\/2000\/svg\" viewBox=\"0 0 32 32\"><path d=\"M10.91 30.41l-2.82-2.82L19.67 16 8.09 4.41l2.82-2.82L25.33 16 10.91 30.41z\"\/><\/svg><\/span><\/button>\n        <div class=\"accordion__content content\" id=\"how-does-my-cap-relate-to-extensions\" aria-hidden=\"true\">\n                    <p>You need to inform your unit coordinators and unit lecturers every time you need to apply for an extension. Please ensure that the additional time has been confirmed by the unit coordinator.<\/p>\n<p>For extensions longer than the additional time supported in the CAP you will have to apply for an Assessment Extension and obtain additional documentation as specified on the form.<\/p>\n<p>Additional time to submit assessments is subject to final approval by the unit coordinator. In some cases, additional time may not be possible (e.g. a group assignment or practical assessment).<\/p>\n<p>Please note that the CAP does not support changes to the dates of tests or exams.
